OF-3422: Validate the client stream header that follows STARTTLS - #3526
Builds on #3525 (OF-2643): it reuses StanzaHandler.refuseStream from that PR, so this PR also shows #3525's commits until that is merged. Only the last three commits are new here.
With xmpp.client.validate.host enabled, the 'to' attribute of a client stream header was checked on the first header only. After STARTTLS the client opens a new stream, and its header was answered with stream features without being checked. RFC 6120 section 5.4.3.3 makes the post-TLS header the one that counts, so a client could name any host there.
Commits, each with its own tests:
Correct the description of client host validation. The class documentation said a valid subdomain of the XMPP domain is accepted, but StanzaHandler.isHostUnknown accepts only the XMPP domain itself. Documentation only; the new tests pin the existing behaviour and pass before and after.
Validate the client stream header that follows STARTTLS. ClientStanzaHandler.tlsNegotiated now validates that header as the first one is, and refuses it with host-unknown when it names a host that is not serviced. A header without 'to' is accepted, as on the first stream.
Compare a client stream host with our domain in any case. StanzaHandler.isHostUnknown compared character for character, so a client that wrote the domain in capitals was refused. It now normalises with JID.domainprep, as the S2S check does. This also affects the 'from' chosen when any stream is refused.
With the setting off (the default), only commit 3 changes behaviour, through that choice of 'from'.
Testing: 12 tests in ClientStanzaHandlerHostValidationTest. Without the change, the refusal tests after STARTTLS and the tests for capitals fail. The net, server, session and websocket test packages pass. LocalIncomingServerSessionTest.incomingTest is skipped, as it is disabled on main (OF-3269).
Server Dialback decided whether it services the domain in a db:result
'to' with its own check. That check matched subdomains with
contains(), so any domain that included the XMPP domain's text could
pass if it had a component route, and it threw a NullPointerException
when 'to' was missing.
Stream header validation needs the same decision, so the rule moves to
ServerStanzaHandler.isLocallyServiced: the XMPP domain, a proper
subdomain with a component route, or a domain for which trunking is
enabled (exact match, as before). A db:result without 'to' is now an
unknown host, as XEP-0220 requires the attribute.
The error path of createSession opens a stream that carries the stream
error, delivers it and closes the connection. Validating the stream
header that follows TLS negotiation needs the same response, so that
code moves to a method of its own. Behaviour does not change; the new
tests pin the existing response, which had no coverage, and pass both
before and after the extraction.
ServerStanzaHandler never validated the 'to' of inbound stream
headers, so a peer that authenticated with SASL EXTERNAL was accepted
whatever domain it addressed. The new property
xmpp.server.validate.host enables the host-unknown check that client
connections already have. It is off by default so that upgrades do
not change behaviour; OF-3421 tracks enabling it by default.
A stream without 'to' stays acceptable, as legacy dialback peers omit
it.
Only the first stream header reached the host check: after STARTTLS,
ServerStanzaHandler.tlsNegotiated re-creates the session directly.
RFC 6120 section 5.4.3.3 requires values from before TLS to be
discarded, which makes the header sent after STARTTLS the one that
counts, and the only one that an active attacker cannot have altered.
When xmpp.server.validate.host is enabled, that header is now checked
in the same way, before the session is re-created.
When a stream header is refused, the stream that carries the error was
opened from the host that the peer addressed. For a host-unknown error
that names a domain this server has just said it does not service, and
when the header had no to attribute the from attribute was omitted.
RFC 6120 section 4.9.1.3 requires an authoritative name for the
receiving entity, or the empty string, in both cases.
The refusal is now opened from the XMPP domain in those cases. Other
refusals still answer from the addressed host, which is serviced. This
applies to client streams refused under xmpp.client.validate.host too.
A refused stream was opened from our own domain only for a host-unknown
error. A stream that names a domain this server does not service, and
that is refused for another reason (such as an invalid namespace while
host validation is disabled), was still opened from that domain.
The refusal is now opened from the XMPP domain whenever the addressed
host is missing or is unknown according to isHostUnknown, whatever the
error, as RFC 6120 section 4.9.1.3 requires. A serviced host that is
addressed is still used. Component streams, whose handler accepts only
the XMPP domain as known, are refused from the XMPP domain too.
Trunking.isTrunkingEnabledFor normalised every value of
xmpp.gateway.domains with JID.domainprep, which throws for a value that
is not a valid domain name. With trunking enabled and one such value in
the list, any check that reached it threw. Server Dialback already used
this method; with xmpp.server.validate.host enabled, the validation of
each inbound stream header now does too, so one bad value would break
incoming server connections instead of refusing them with host-unknown.
An invalid configured value is now logged and ignored, and the other
configured domains are still recognised.
When a stream is refused, the stream that carries the error declares
the content namespace of the refused header. A header without a default
namespace made dom4j throw a NullPointerException there, so the peer
was sent no stream error. That was reachable on the first header (an
invalid-namespace refusal), and with xmpp.server.validate.host enabled
it is reachable on the header that follows STARTTLS too.
Such a header is now answered in the namespace of the handler.
The new test classes set properties such as xmpp.server.validate.host
and the trunking configuration, and clear them before each test, but
left the values of their last test in place for whichever test class
runs next in the same JVM. They now clear them after the class too, as
SaslOutcomeTest and StanzaHandlerTest do.
Also rewords the description of one test that read awkwardly.

Walkthrough
Server-to-server host validation is configurable and checks whether addressed domains are serviced. Client and server stream handlers also validate restarted stream headers after TLS negotiation. Shared domain checks support server dialback, while stream refusals select a response host and content namespace. Tests cover domain recognition, refusal behavior, and validation before and after TLS.

Every check of a trunked domain normalised each configured value of
xmpp.gateway.domains again. With an invalid value in the list, that
logged a warning, with a stack trace, for every inbound stream header
and dialback request that reached it (reported by CodeRabbit on igniterealtime#3525).
The normalised domains are now kept until the configuration changes, so
an invalid value is reported once per change, without a stack trace,
and each check is a set lookup. A change to the configuration still
takes effect on the next check, which a new test verifies.
The shared rule normalises a domain before deciding whether it is
serviced, so Server Dialback now accepts a db:result whose 'to' names
the XMPP domain in another case, which its old, case-sensitive check
refused. The value was then used as received: the check for an existing
session for the same domain compared it case-sensitively, so a second
session was not recognised as a duplicate, and the result was reported
from the domain as the peer wrote it (reported by CodeRabbit on igniterealtime#3526).
Once the domain is accepted, validateRemoteDomain continues with its
normalised form and records it in the request, which
LocalIncomingServerSession.validateSubsequentDomain reads to report the
result. The permission and certificate checks that precede and follow
it are stubbed in the new tests, as they read the database and the
peer's certificates.
With xmpp.server.validate.host enabled, a trunk refuses inbound
connections for domains it does not service. An administrator who
enables it on a trunk needs to know that the trunked domains are still
accepted, and that their subdomains are not.
The documentation of xmpp.client.validate.host said that the to
attribute of a client stream header has to match the server name or a
valid subdomain. StanzaHandler.isHostUnknown, which ClientStanzaHandler
uses, accepts only the XMPP domain itself, so a client stream that names
a subdomain is refused. The description now says so, and the comments
at the two places that perform the check refer to isHostUnknown, as
the hosts that are serviced differ per type of stream.
The new tests pin the behaviour that is described: they pass before
and after this change, which alters no code.
With xmpp.client.validate.host enabled, the to attribute of a client
stream header is checked only on the first header. After STARTTLS the
client opens a new stream, and its header was answered with stream
features without being looked at. RFC 6120 section 5.4.3.3 requires
values from before TLS to be discarded, which makes the header after
STARTTLS the one that counts, so a client could name any host there.
That header is now validated in the same way, and refused with
host-unknown when it names a host that is not serviced. This mirrors
the check for server-to-server streams added by OF-2643.
With xmpp.client.validate.host enabled, the to attribute of a client
stream header was compared with the XMPP domain character for
character, so a client that wrote the domain in capitals was refused
with host-unknown, although domain names are case-insensitive. Server
streams (OF-2643) already compare after normalisation.
StanzaHandler.isHostUnknown now normalises the host with
JID.domainprep before comparing it, as ServerStanzaHandler does. A host
that is not a valid domain name remains unknown. This applies to the
first header and to the one that follows STARTTLS, and to the choice of
the from attribute when a stream is refused.
